Please print and complete the following medical history form to the best of your ability. Bring it with you on the date of your visit, along with any relevant health documentation (e.g., reports of prior surgeries, CT or MRI results, etc.).
NOTE: Please bring recent CT or MRI images with you (preferably on CD) so that they may be reviewed personally by our physicians.
SINUS QUESTIONNAIRE
The Sinonasal Outcome Test (SNOT-22) is a validated questionnaire that is widely used in research and clinical practice. It helps us to better characterize your sinonasal symptoms. Please print a copy of it, complete it, and bring it with you to your initial visit.
